As a UBH member you may initiate the pre-treatment eligibility and treatment assessment process by calling the MH/SA telephone number on the back of your UBH INS member's card. When calling, you should be very specific and ask for (1) Autism Services and (2) You need to speak with an Autism Care Advocate. UBH has three dedicated teams of care advocates (or case managers) who assist members with Applied Behavior Analysis (ABA) Autism benefits. Please remember that all ABA services require pre-authorization. United also has a Web site to answer additional questions, provide forms, and explain their service delivery model.
